Event Description
It was reported that the stent moved on balloon.A 4.00 x 38 synergy drug-eluting stent was selected to treat a lesion in the proximal right coronary artery (rca).However, upon opening the package, the stent was damaged and hanging halfway off of the balloon.The device was not used, and the procedure was completed with another of the same device.There were no patient complications reported in relation to this event.
