It was reported that the cleaning brush gets caught brush stuck in channel while using the gastro intestinal scope.The issue was found at reprocessing.There was no patient harm associated with the event.

The device was returned and the evaluation found no malfunctions aside from the allegation reported in b5.Should additional relevant information become available, a supplemental report will be submitted.
