A field service engineer (fse) was at the customer¿s site to address the reported event.Fse confirmed the complaint by reviewing the error logs.The fse was not able to reproduce the error code onsite but suspected the stc lane plastic chain or motor home sensor.Fse replaced the stc lane plastic chain and the pip board for the mixing motor home sensor.In addition, fse performed the mixing motor in mainte function for fifteen (15) minutes turning it off and on several times and no more errors reoccurred.Fse repaired and validated the analyzer by successfully performing quality control run without error and within acceptable range.No further action required by field service.The aia-2000 analyzer is functioning as expected.A 13-month complaint history review and service history review through aware date of event for similar complaints was performed for serial number 10661001.There were no similar complaints identified during the searched period.Aia-2000 operator's manual on the appendix 4: error messages: (4133) stc lane mixer home overrun.Cause: the home sensor activated improperly after movement of the stc lane mixer.If retry fails, the measurement result will be flagged (mf flag).Solution: contact tosoh service center or local representatives.The most probable cause of the reported event was due to the faulty stc lane mixer home overrun and pip board.
 
Event Description
A customer reported error message ¿4133 stc lane mixer home overrun¿ on the aia-2000 analyzer.The analyzer is down.A field service engineer (fse) was dispatched to address the reported event which resulted in a delayed reporting of patient samples for beta human chorionic gonadotropin (bhcg).There is no indication of any patient intervention or adverse health consequences due to the delay in reporting of patient results.
 
Manufacturer Narrative
The pip electric board and stc lane mixer plastic chain were returned to the instrument service center on (b)(6)2024.A visual inspection was performed on the returned parts and no damage was observed.Functional testing was performed using a digital multimeter found an open circuit for the stc lane mixer plastic chain, which indicates an intermittent continuity failure.The most probable cause of the reported event was due to the faulty pip electric board and stc lane mixer plastic chain.
