During an atrial fibrillation a polarx catheter was selected for use.A c-map was evaluated before moving to the right inferior pulmonary vein and it was not really strong.The recorder had to gain up all the way to max and the phrenic stim was still not very strong, but it was decided to ablate.The catheter was placed, and the freeze was started.Upon phrenic and the physician had it, but it wasn't very strong.At almost the 2-minute mark the phrenic just bottomed out and stopped.A double stopped was preformed immediately on ablation.The catheters were moved around trying to regain the phrenic back and medication was administered.The phrenic did not return and the procedure was cancelled.The patient was taken to post op with phrenic nerve palsy.The device is not expected for return due to disposal.
