Event Description
Device report from japan reports an event as follows: it was reported that an unknown surgery was performed on (b)(6) 2024.L5/s fusion, verse 1 intervertebral.The cage is a competitor¿s product.The procedure was completed successfully with no surgical delay.After the original procedure, on (b)(6) 2024, the sales rep was informed that the set screw in question had backed out, and the l5 left set screw had come off.The cause is unknown, a revision procedure was scheduled for (b)(6) 2024.No further information is available.This report is for an expedium verse spine system unitized set screw.This is report 1 of 2 for (b)(4).

The patient status and outcome were reported as revision surgery having been performed.
