Event Description
It was initially reported, the duodenovideoscope exhibited a scope connective error.Additional information was later received indicating that the issue occurred at the beginning of a therapeutic endoscopic retrograde cholangiopancreatography (ercp) procedure.The patient was under anesthesia at 1545 and the procedure did not start until 1723 as an additional scope had to be retrieved and processed from another campus.The procedure then lasted 57 minutes and completed with the new scope without further incident.There were no reports of patient harm.This new information received from the customer changed the initially reported event to a serious injury.
