A physician reported a codman perforator (id 261221) disassembled when pulling the product from cranium after making the first burr hole.All the disassembled parts were recovered and the device was used to complete the surgery.A total number of burr hole made by the product was one and the drill used with the perforator was aesculap/elan 4 electro.There was less than 30 minutes delay in the procedure.The procedure performed was craniotomy for chronic subdural hematoma.According to information provided, it is unknown if the recommended spring tests were being performed between each burr hole.It is also unknown if the perforator clicked in place in the drill.
