Event Description
During an atrial fibrillation procedure, an x-ray image revealed the needle perforated the sheath when performing the transseptal puncture.However, a guide could still be inserted in the patient's left atrium.An agilis was used to perform the procedure (which had been planned to perform the ablation) with no consequences to the patient.An ett (exercise tolerance test) was performed at the end of the procedure and did not show an effusion or any unexpected / abnormal element.

The device was not returned.Two images were submitted for evaluation to product performance engineering; no device components were returned.The photos appeared to show a swartz braided introducer.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.The cause of the reported sheath puncture remains unknown.
