H.11 additional manufacturer narrative: the aquabeam handpiece was returned for investigation.The aquabeam handpiece was tested with the associated aquabeam motorpack triggering e22 and e23 error messages and could be cleared.Additionally, the handpiece lower shell and pcb were swapped with a known good one and the handpiece was tested with the related motorpack.Three docking cycles and an entire simulated aquablation session were performed successfully without triggering any errors, indicating a faulty pcb.Root cause is pcb failure and root cause source is undeterminable.A review of the device history record (dhr) for the aquabeam robotic system/serial number (b)(6) and aquabeam handpiece/ lot number 23c04637 was conducted, which confirmed that there were no non-conformances, failures, discrepancies, or missed steps during the manufacturing process that could be related to the reported event.The review indicated that the device met all design and manufacturing specifications when released for distribution.The current user manual sj-um0101-00 rev.B, um, aquabeam robotic system user manual, us, baytech was reviewed.The aquabeam robotic system user manual, um0101 rev.F, states the following: table 5: system detected errors and faults e23 - motorpack error release foot pedal and click x.1) if error persists, reconnect handpiece to motorpack.2) if error continues, replace handpiece submission of this report does not constitute an admission that the manufacturer's product caused or contributed to the event.
