|
Philips received a complaint from the customer, reporting a data acquisition (da) printed circuit board assembly (pcba) analog to digital converter (adc) failed message occurred on the v60 ventilator.The device was not in clinical use.There was no report of harm.There was no patient involvement.The device did not meet specification for intended use and was removed from service.A philips remote service engineer (rse) evaluated the issue with the biomedical engineer (bme) and confirmed diagnostic codes 111c and 1006 in the device log, both indicating a da pcba adc failure.The rse assisted the bme in troubleshooting the cause and found the da to motor control (mc) cable was not fully seated at the da pcba.The cable connection was re-established, and the device returned to normal function.The device was operational and returned to service after repairs were completed.The investigation concludes that no further action is required at this time.If additional information is received the complaint file will be reopened.
