Event Description
|
Medtronic received information regarding an imaging system being used during a sacroiliac and thoracolumbar procedure.It was reported that images were not transferring from the imaging system to the navigation system.The system transferred images the first two spins, but would not transfer the third.Troubleshooting confirmed the correct navigation was selected.Confirmed the navigation displayed 3 green boxes and the navigated check box on the pendant was checked.The scan did not have the navigation tag.They took a re-spin.There was no reported impact to patient outcome and no reported delay.

Event Description
|
Additional information was received.Staff reported that during the 3rd and 4th spin of a t10 to pelvis posterior spinal fusion (psf), the imaging system started making a grinding noise during exposure, seemed to get stuck, the resulting spin would not transfer to the navigation system, and that the image quality and accuracy have been questioned by the surgeon.They reported seeing "double" or "duplicate" pedicles on the (4th) spin in question.Navigation was aborted and they proceeded with the case using fluoro guidance only.
