User facility medwatch received that stated that the patient was on warfarin for anticoagulation and presented to an outside hospital with left sided weakness and numbness.A head computed tomography scan revealed a large right intraparenchymal hemorrhage with intraventricular extension.Their international normalized ratio was 2.2.They were transferred to a different hospital for further care.After a neurology evaluation and discussion with family, the patient was made comfort measures only.
