|
It was reported that a spinning spiros® closed male luer, red cap disconnected during patient use.The report stated that they had a large chemo spill due to a spiros coming disconnected from the tubing.The spiros came disconnected from the tubing and chemo spilled onto the patient.The reporter was planning on ordering the new ca-1 clip once it is available, however, now it is being questioned why the piece needed to be developed.There is a concern for the safety of their staff and patients.The lot number of the spiros that failed is unknown.As there are multiple lots stocked in the chemo hood.There was no patient harm reported.
