The device was returned to olympus for inspection, and the customer's complain was confirmed, due to damage on charged coupled device (ccd) unit, the image disappears during angulation in up/down directions.In addition, the following non-reportable malfunctions were found during the device evaluation: due to damage on insertion tube rotation ring, connecting tube does not rotate smoothly and connecting tube has a buckling.Should additional relevant information become available, a supplemental report will be submitted.Note: e2 was marked inadvertently, it should remain blank.
 
Event Description
It was reported, the  bronchovideoscope exhibited an image loss when the distal end is angled.The issue occurred during an unknown event.There were no reports of patient harm. .
