The healthcare professional reported that during an endovascular embolization procedure, the 4mm x 16mm enterprise 2 vascular reconstruction device (vrd) (encr401612 / 8129990) was impeded in the distal end of the 150cm x 5cm prowler select plus microcatheter (606s255x / lot# unknown) and could not pass through the microcatheter.The physician retracted the stent and made another attempt to deliver the stent, but the same issue was encountered.The physician removed both devices and replaced them; a competitor brand microcatheter was used to complete the procedure.The devices are reported as not available for return.There was no report of any negative patient impact.On 12-mar-2024, additional information was received.Per the information, the patient is a 66-year-old female.The target vessel was the posterior communicating artery.When the stent was removed, it was still on the delivery wire.The stent / stent delivery system did not appear damaged.Nothing unusual was noted about the system prior to use.A continuous flush had been maintained through the microcatheter.The replacement stent was another 4mm x 16mm enterprise 2 vascular reconstruction device (encr401612).The reported issue did not result in any procedure delay.
