This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Corrected fields: d8 additional information added to field h6 and h3.Since the device is more than 15 years old, the device history record was unable to be reviewed.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.Based on the results of the investigation, the strands of the forceps elevator wire were cut by fatigue breakdown due to repeated operation of the forceps elevator.Additionally, the forceps elevator wire became frayed and raised due to repeated brushing around the forceps elevator and repeated attaching/detaching of the distal cover.The suggested event can be detected by handling the device according to the following ifu: - inspection of the forceps elevator mechanism the suggested event can be prevented from occurring by handling the device according to the following ifu: - using a stiff brush or excessive force when brushing may scratch the distal end and result in water leakage; cause the elevator wire to come off the distal end, bend or kink the elevator wire so that the forceps elevator will no longer work.Olympus will continue to monitor field performance for this device.
