It was reported that during a lead revision procedure, (see mfr.Report 3006630150-2024-01644) the set screw of the implantable pulse generator (ipg) was backed out and the lead was unable to be released from the ipg.It was noted that the avista lead was pulled out of the implantable pulse generator (ipg) header with no problem.After the set screw was opened too far, the screw came out.Even when the screw was not in the header anymore, and the physician pulled very hard, it was not possible to pull out the linear lead.This resulted in the lead being demolished, and therefore neither the lead nor the ipg could be used any longer.In addition, the avista lead had to be removed and exchanged as it was not possible to insert a stylet into the avista lead in order to push it back in place.Therefore, a new ipg and two new linear leads were implanted.The patient was doing fine postoperatively.
