Event Description
It was observed that during the device inspection, the duodenovideoscope exhibited a crack in the adhesive between the distal end and the server plug-in, foreign objects in the adhesive around the light guide lens, and foreign material inside the light guide lens.There were no reports of patient involvement.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the device evaluation and the legal manufacturer's final investigation.New information added to the following fields: h3, h6, h10.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 11 years since the subject device was manufactured.Based on the results of the investigation and the information provided for the phenomena of " distal end, light guide-lens glue and inside light guide-lens has foreign material", it could not be determined what the foreign material was.There was no damage to the area where the foreign material was detected, the user facility reported that the device was reprocessed according to the ifu; however, the detailed steps were not provided.Therefore, the cause of the material remaining in the device could not be determined.The event of "distal end has foreign material" can be detected/prevented by following the instructions for use which state: operation manual_ preparation and inspection_ inspection of the endoscope inspect the external surface of the entire insertion tube including the bending section and the distal end for dents, bulges, swelling, peeling, scratching, holes, sagging, transformation, bends, adhesion of foreign body, dropout of parts, any protruding objects or other irregularities.Operation manual_ important information ¿ please read before use_ warnings and cautions.Warning: do not strike, bend, hit, pull, twist, or drop the endoscope¿s distal end, insertion tube, bending section, control section, universal cord, or endoscope connector with excessive force.The endoscope may be damaged and could cause patient injury, such as burns, bleeding, and/or perforation.It could also cause parts of the endoscope to fall off inside the patient.In addition, the event of " light guide lens glue has foreign material" and "foreign material inside light guide lens" can be detected/prevented by following the instructions for use which state: detection method is described in tjf type q180v operation manual 3.3 inspection of the endoscope 3.8 inspection of the endoscopic system preparation and inspection.Preventive measure is described in tjf type q180v reprocessing manual 5.4 manually cleaning the endoscope and accessories.Olympus will continue to monitor field performance for this device.
