Event Description
It was reported the stent was clogged inside the forceps channel of the duodenovideoscope.The issue occurred during therapeutic endoscopic biliary drainage (ebd) procedure.The procedure was completed with the same device after removal of the stent.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 6 years since the subject device was manufactured.The device was returned to olympus for inspection, and based on the results of the investigation, the customer's allegation was confirmed.The issue could have occurred due to the tube was kinked in the channel.However, the root cause could not be specified.The legal manufacturer was unable to confirm whether the customer's reprocessing steps were deviated from the instructions for use.It was confirmed that the attached documentation for the stent product states that the stent is not to be retrieved via the endoscopic lumen.Olympus will continue to monitor field performance for this device.
