It was reported that during a surgical procedure, this cardiac resynchronization therapy defibrillator (crt-d) device delivered three inappropriate shock therapy due to oversensing noise signals.The hcp noted that a magnet was used to prevent the device from oversensing environmental noise signals during the procedure had inadvertently moved, causing the crtd device to over sense the noisy signals which led to the inappropriate shock therapy.At this time, the device remains in service.No additional adverse patient effects were reported.Subsequent additional information was provided that stated the crt-d device was interrogated after the surgical procedure was completed by the physician.It was noted that the magnet was used throughout the procedure.The device appears to be operating as expected.The physician will continue with monitoring the device.No additional adverse patient effects were reported.The device remains in service.
