It was reported that high out of range pacing impedance and high out of range high voltage (hv) impedance was observed on a remote transmission.The patient was brought into the clinic to be evaluated further.In the clinic, it was confirmed all three leads had an out of range impedance, no capture, and no sensing.Therapies were disabled and pacing was turned off in the clinic.X-rays showed the leads were in the original implant location.The patient was brought into the lab to test leads, and all leads were tested within normal limits with no abnormalities present.A new generator was implanted and the chronic leads connected to the new generator in working condition.A device/header malfunction was suspected.The patient was stable.
