Device evaluated by manufacturer: the device was returned for analysis.A visual tactile inspection, microscopic analysis and device to device interaction test was completed.The inner polymer extrusion was damaged 5.8cm from the tip of the device.No damages or issues were noted to the balloon or hypotube shaft.No tears or pinholes in the balloon material.All blades were fully bonded on the balloon and did not exhibit any signs of damage.Tip showed no signs of tip damage.Microscopic examination of the proximal and distal markerbands identified no damage.The device could not be tracked on a 0.0140" test guidewire.The stretching of the inner polymer extrusion prevented the guidewire to pass through.
 
Event Description
Reportable based on device analysis completed on 29feb2024.It was reported that the wire could not cross.A 6mmx4.00mm wolverine coronary cutting balloon was selected for use.It was noted that the non-boston scientific guidewire was unable to cross into the wolverine.The procedure was then completed with a different device.No complications reported and there was no patient injury.However, the inner polymer extrusion was damaged.
