Event Description
It was reported that there was a separation between the female connector (navy tip) and main body (baby blue) of the minicap transfer set.The event was further described as ¿navy blue tip loose from the baby blue portion of transfer set¿.This occurred when disconnecting after a peritoneal dialysis catheter flush.Alcavis disinfectant had been used on the set.The transfer set was replaced.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
