This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Additional information was added to fields d10, h3, h4 and h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 2 years since the subject device was manufactured.The olympus technical assistance center (tac) performed troubleshooting with the customer, and it was determined that the maj-1941 light control cable was loose.Reconnecting the cable resolved the issue.Three attempts were performed to obtain additional information regarding the patient and event information, but no response to the questions were provided.The customer did confirm that the issue was resolved with the tac troubleshooting provided.Based on the results of the investigation, it is presumed that the reported issues resulted from the light control cable maj-1941 being loose.However, a specific root cause could not be identified.Olympus will continue to monitor field performance for this device.
