Event Description
According to the reporter, during the laparoscopic single anastomosis gastric bypass,  on the very first firing, while on pouch creation, transection of the stomach at the lesser curve.The surgeon fired the reload across the stomach.The surgeon had great difficulty removing the reload from the stomach whilst pulling on the black knob.The black knob has been able to partially pull back the i beam.The reload was then stuck.The surgeon simply waited for the tissue to disperse then was able to pull back the black knob and remove the reload.There was no patient injury.
