Event Description
It was reported that a patient underwent a pulmonary vein isolation (pvi) ablation procedure with a thermocool® smart touch® sf bi-directional navigation catheter.The physician wanted to push the catheter in the left atrium.Before this step, he wanted to flush the catheter and he realized that there comes no water out of the tip.They tried everything but the catheter was blocked.The water from flushing did not come out of the holes of the catheter even if they saw that the tubing kit and the pump was flushing correctly.They used a new catheter and the new catheter was able to get flushed normally.The procedure continued.No patient consequences were reported.
 
Manufacturer Narrative
The bwi product analysis lab received the device for evaluation.The analysis has begun but is not completed at this time.When the investigational analysis has been completed, a supplemental 3500a report will be submitted.This report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date.This report does not reflect a conclusion by biosense webster, inc., or its employees that the report constitutes an admission that the product, biosense webster, inc.Or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.Manufacturer's reference number: (b)(4).
 
Manufacturer Narrative
Per internal review on 1-apr-2024, it was determined that this event is not mdr-reportable.Bwi received information on 19-mar-2024, that indicated that no ablation occurred with the complaint device.This device was not used on the patient at all and therefore presents no patient risk.No further reports will be sent regarding this event.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's reference number: (b)(4).
