Event Description
It was reported that there were errors e216 and b30 scope communication errors while using the lightsource.There was no delay or patient harm associated with the event.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.New information added to the following fields: b5, d8, d9, h3, h4, h6 and h10.Please disregard e3 as it was inadvertently selected and cannot be removed.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 7 years since the subject device was manufactured.The device was returned to olympus for inspection, and the customer's complaint was not confirmed.Based on the results of the investigation, a definitive root cause could not be determined.However, it is like that the event reported as "b30 error" was caused by a abnormality occurs in the communication between the endoscope and the processor.The event can be detected/prevented by following the instructions for use which state: instructions otv-s190 chapter 9 troubleshooting 9.2 troubleshooting guide olympus will continue to monitor field performance for this device.
