It was reported that the patient had a centrimag right ventricular assist device (rvad) placed at the time of left ventricular assist device (lvad) implantation.When the patient was being weaned from centrimag, the doctor reported that the centrimag pump clotted.Per the cardiologist, the patient was not on unfractionated heparin (ufh), and international normalized ratio (inr) was 1.6 (receiving warfarin).Given that it was in weaning phase, flows were low in 1-1.5 l/min range.The patient was not harmed and was currently stable.The site additionally stated that the rvad was planned and their lvad operated as expected and did not cause or contribute to the event.The site did not use adequate anticoagulation at anticipated lower flows while weaning the rvad.
