Event Description
|
It was reported that during a pulmonary vein isolation (pvi) procedure to treat an atrial fibrillation (afib), an intellamap orion high resolution mapping catheter and a farawave catheter were selected for use.The orion catheter was used for premap and worked perfectly.Ablation was done with the farawave catheter and then the orion catheter was inserted back into the sheath and deployed into basket mode.A map was made of the right pulmonary veins and then the physician noticed the slider for deploying and undeploying the orion catheter no longer worked.The physician felt unsafe, so catheter was replaced, and they were able to finish mapping.The procedure was able to be completed successfully without any patient complications.The orion catheter is expected to be returned for analysis.
