|
It was reported that during set-up of the device for a cardiopulmonary bypass procedure, the roller pump occlusion mechanism was stuck in the open position.Perfusionist (ccp) was not able to turn occlusion knob.As a result, an alternate device was employed.The surgical procedure was completed successfully.There were no delays, no blood loss, or no adverse consequences to the patient.

Per the laboratory evaluation, the technician observed the pump occlusion was stuck upon receipt of the pump.The occlusion was able to be loosened after removal of the occluder knob and tongue, and functioned after the occluder knob was placed back on the pump.Abnormal wear was seen on the screw adjust slot where the tongue fits.
