It was reported that a minicap extended life pd transfer set was unable to secure to a non-baxter adapter.This occurred during patient use.There was no patient injury or medical intervention associated with this event.No additional information is available.

(b)(4).Concomitant medical products - the reporter stated that the adapter was manufactured by either fresenius or quinton.A request for the return of the device has been made.Should additional relevant information become available, a supplemental report will be submitted.
