Event Description
Ppd and medical records received.Patient was revised for a painful left hip.The revision operative note indicated the cup was loose.This was a metal on metal hip and there was not mention of metallosis.

Event Description
Medical records and sticker sheets received.After review of medical records, the patient was revised to address pain, lucency around the acetabulum and was increasingly dependent on crutches.Operative findings include: a very thick layer of scar tissue, blackish appearing synovium, a grossly loose cup and a thick fibrous membrane behind the acetabulum.
