|
It was reported to boston scientific corporation that a stone cone nitinol urological retrieval coil was used during a transurethral lithotripsy procedure performed on (b)(6) 2014.According to the complainant, during the procedure, a stone cone was inserted through a 6fr ureteroscope and passed beside an impacted ureteral stone.After the device was deployed, approximately 16 centimeters of the distal portion of the device, including the coil, separated and migrated towards the kidney.The physician disintegrated the stone and retrieved the broken part of the device using a ureteroscopy grasper.The procedure was completed with another stone cone nitinol urological retrieval coil.There were no patient complications reported as a result of this event.The patient's condition at the conclusion of the procedure was reported to be stable.

The complainant was unable to report the upn and lot number; therefore, the manufacture date and expiration date are unknown.However, the complainant stated that the device was used prior to the expiration date.(b)(4).The complainant indicated that the device will not be returned for evaluation; therefore, a failure analysis of the complaint device could not be completed.If any further relevant information is identified, a supplemental medwatch will be filed.
