It was reported from (b)(4) that during an unspecified surgical procedure, it was observed that the universal chuck device did not function.According to the report, the ring of the device was bent and coming loose.It was further reported that the device appeared to be ¿hammered upon¿.The reporter stated there were a lot of scratches and small dents on the device.There were no delays to the surgical procedure as a spare device was available for use.There was patient involvement.There were no reports of injuries, medical intervention or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.

The actual device was returned for evaluation.Reliability engineering evaluated the device and observed that the device was worn out.Therefore, the reported condition was confirmed.The assignable root cause was determined to be due to wear from normal use and servicing.If additional information should become available; a supplemental medwatch report will be sent accordingly.
