Event Description
A third party biomed reported that their device was only delivering a monophasic shock instead of a biphasic shock, during testing.With this reported issue, the defibrillator output energy could be reduced by up to approximately 20% from the selected energy level.There was no report of patient use associated with the reported issue.
 
Manufacturer Narrative
Physio-control continues to investigate the reported failure and will submit a supplemental report on this event to the fda as provided by 21 cfr 803.56.

The customer contacted physio-control and advised that during testing, the device was improperly connected to their defibrillation analyzer.This improper connection caused the reported issue and there was no issue with the device.Normal functionality of the device was confirmed through functional and performance testing and the device has since been returned to service for use.The device was not returned to physio-control for evaluation.
