Event Description
A scrub technician reported that as the surgeon was performing a vitrectomy surgery of the left eye, the illuminator and probe got stuck in the trocars.The scrub technician tried to hold the trocars in place for the surgeon, but the trocars dislodged and the sclerotomies leaked.The surgeon used sutures to close the sclerotomies.The scrub technician obtained new trocars and new sclerotomies were created.The surgery was completed.No additional information is expected.
 
Manufacturer Narrative
Investigation including root cause analysis is in progress.A supplemental mdr will be filed as necessary in accordance with 21 cfr 803.56 when additional reportable information becomes available.(b)(4).
