It was reported that during set-up of the device for a cardiopulmonary bypass procedure, the cooler heater unit was tripping the line isolation monitor (lim) in operating room (o/r) #5 as soon as it was powered on.As a result, an alternate device employed. The surgical procedure was completed successfully. There was no delay, no blood loss, nor adverse consequences to the patient.

Per the field service rep (fsr), the ccp confirmed the issue was the cooler heater unit by plugging it into a few different independent circuits and it always tripped the lim.In house biomedical engineers (biomeds) eventually confirmed the leakage current was too high.Other than the high leakage, the tcm was operating normally. The fsr confirmed the leakage current was too high to register on the safety analyzer. The fsr disconnected confirmed the high leakage was coming from the 850 watt heater. The fsr opened and cleaned the gasket surfaces and internal s-flow chamber. The fsr installed a new 850 watt heater. As a preventative measure, he also replaced the 700 watt heater and a new gasket. He reassembled the unit and completed a full inspection. The leakage current is now within the specifications. The unit operated to manufacturer for further evaluation. If additional info becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
