(b)(4).The complaint rt266 breathing circuit was not returned to fisher & paykel healthcare in (b)(4) for evaluation.Our investigation is based on our knowledge of the product and the information provided by the hospital.The hospital reported that the breathing circuit passed the initial leak test and that the wye detached while the breathing circuit was being handled.The hospital reported that no damage was observed on the wye piece.Without the return of the complaint device we are unable to determine what may have caused the reported event.All rt266 infant dual-heated evaqua2 breathing circuits are visually inspected and pressure and flow tested during production, and those that fail are rejected.The customer has confirmed that the subject breathing circuit was in use before the event occurred, which indicates that the circuit became damaged during use.The user instructions that accompany the rt266 state the following: - "check all connections are tight before use." - "perform a pressure and leak test on the breathing system and check for occlusions before connecting to a patient." - "set appropriate ventilator alarms.".
