It was reported that during set-up of the device for a cardiopulmonary bypass procedure, the cooler heater unit was leaking water.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.

Per the field service rep ( fsr) verified the water leak from the rear panel of th unit.The fsr found the cardioplegia (cpg) pump outlet coupling was cracked.The fsr replaced the cracked coupling, as well as the cpg pump housing and 1/2" connector.The fsr tested operation and verified no water leaks.The unit operated the mfr specifications and was returned to clinical use.The suspect parts were returned to the mfr for further evaluation.If additional info becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
