Patient was revised to address pain.No device associated with this report was received for examination.A worldwide complaint database search found no other reported incident(s) against the provided product/lot combination(s) since release for distribution.Based on previous investigations this complication of joint replacement is unlikely to have been the result of a device failing to meet required specifications.The information received will be retained for potential series investigations if triggered by trend analysis, post market surveillance, or other events within the quality system.Corrective action was not indicated.

Event Description
Pfs alleges elevated metal ions.After review of medical record for mdr reportability, patient was revised to address malpositioned acetabular component with periacetabular ostial lysis and surrounding metal-on-metal reaction.On (b)(6) 2015, it was noted that the cup was pre-much verticel almost 80 degrees and anteverted only a few degrees.This was then extracted.Perry articular ostial lysis in zone 2 with osteolytic membrane surrounding the periphery of the shell was noted as well.This had significant dome lysis and very poor bone stock absence of anterior column.
