Event Description
It was reported that during a difficult coronary sinus cannulation procedure using many guides and electrophysiology catheters, the right atrial (ra) lead became dislodged.There was difficulty advancing and removing the lead so it was explanted and replaced.No patient complications have been reported as a result of this event.
