Event Description
During review of the patient's clinic notes , it was identified that a programming anomaly occurred.It is suspected that the system diagnostic test performed on (b)(6) 2015 had faulted and changed the patient's vns settings to unintended settings.It is unknown if final interrogation was performed on (b)(6) 2015.Manufacturer labeling indicates to perform a final interrogation to confirm intended settings prior to patients leaving the clinic.Upon initial interrogation on the subsequent visit, faulted settings were observed.
