Event Description
|
It was reported that during a cryo ablation procedure when the sheath was in the left atrium of the patient, the stopcock detached from the sheath extension while flushing and aspirating the sheath.The sheath was replaced and the procedure was completed with cryo.No patient complications have been reported as a result of this event.

Product event summary: data files and a photo were returned and analyzed.The data files did not show any system notices on the re ported event date.The pictures provided by the facility demonstrate the stopcock distal end luer was completely detached from the side port tube proximal end.No product was returned for investigation.
