Event Description
It was reported that a minicap transfer set had a defective rotatory clamp which had been over-rotated and did not close correctly.The patient may have applied excessive force and twisted the clamp in the wrong direction.The patient had used an unspecified device to open the set and had cleaned the transfer set with octenisept.There was no patient injury or medical intervention associated with this event.No additional information is available.
