There has been a previous report received where stalling has caused file separation.Since separation of a file could necessitate medical/surgical intervention to preclude permanent damage to a body structure or permanent impairment of a body function, this malfunction would be likely to cause/contribute to a serious injury should it recur.As such, this event meets the criteria for reportability per 21 cfr part 803.The device is available for evaluation, though has not been returned as of this report.Evaluation results will be submitted as they become available.
 
Event Description
In this event it was reported that an e3 motor was running slower and feels like it is not turning the files like usual.The doctor did some trouble shooting and then reported that the motor is now stalling.No patient injury occured.
 
Manufacturer Narrative
The device has been tested for torque and speed acording to our internal standards and fullfills all the acceptance criteria.The problem was caused because attached to the device was a w&h contra-angle 16:1.It's clear that with such ratio the speed become slower while increases the torque.It is recommended to tell the dentist to read carefully the dfu and to use only sirona 6:1 contra-angles as written in the dfu.
