It was reported that the patient underwent an umbilical hernia procedure on (b)(6) 2015 and mesh was implanted.During the placement, it was noticed that the mesh was torn down.It was also reported that the procedure was completed and there were no adverse patient consequences.

The actual sample was returned for evaluation.Visual examination revealed the partial separation of the wings at the welding with the load ring, which may occur during handling and does not have any effect on the product properties and functionality.
