This initial report was identified as a late submission during a two year retrospective review of complaints and mdr¿s following receipt of an untitled letter issued by the fda.(b)(4).The catalog number is unknown and the correct lot number was not provided therefore the date is unknown.The device was not return for evaluation.
 
Event Description
The customer reported during initial calibrations on the unit and the limit knob didn't seem to work correctly and felt like it was not turning correctly.Then the knob came off.Customer unable to place knob back on unit.
