On (b)(6) 2006, the patient underwent another spine fusion surgery on the cervicothoracic region from c6-t3.The rhbmp-2 collagen sponge was used to fuse more than one level of the spine.The rhbmp2 collagen sponge was placed outside a cage over the posterior elements.Following revision surgery, patient's post-operative period has been marked by a period of improvement, followed by pain in the back of his head, radiating to his neck, intermittent dizziness, bilateral arm pain, and numbness and tingling in his hands.Severe pain and symptoms ultimately compelled patient to undergo a second, risky, painful and costly revision surgery on (b)(6) 2011.Despite two revision surgeries, patient experiences constant neck pain and swelling, and pain radiating to shoulders.He experiences difficulty swallowing, periodic choking, and anxiety due to fear of choking.
