Event Description
A surgeon reported during a laser assisted cataract procedure after hydro dissection, the laser fragmentation cylinder pattern seemed to separate and float up anteriorly.During cortex removal, the surgeon noticed the perfect shape of the cylinder cut into the posterior capsule; anterior vitrectomy was performed, and the intraocular lens (iol) was implanted into the sulcus.The surgeon noted the patient has not had any further complications, and post-op care has been unremarkable.
 
Manufacturer Narrative
A reviewed the optical coherence tomography (oct) scans was performed no issues were found.The investigation did not identify any system issues that would indicate that the laser contributed to the reported event.There were no technical services requested by the customer.The device history records (dhr) for the device was reviewed.No abnormalities that could have contributed to this event were found.The associated device was released based on company acceptance criteria.The root cause of the reported event cannot be determined conclusively.The manufacturer internal reference number is: (b)(4).
