Should any additional information be received by the customer then an additional report will be submitted.(b)(4).The carefusion field service representative (fsr) evaluated the unit along with the involved patient circuit and found that the green pressure line was loosely attached to the cap diaphragm.This is a part of the circuit that is assembled by the user prior to patient-use.Once the fsr tightened the attachment, the unit pressurized correctly.
 
Event Description
The customer reported that this 3100a high frequency oscillating ventilator made a "loud squeal", all panel meters went to "zero", and the driver shut off while on a patient.There were visual and audible alarms present.The end-user was unable to re-pressurize the system and alternate ventilation was provided by replacing the involved unit with a known good unit.The customer stated that there was no injury or harm associated with this reported issue.
